If accepted for this trip, I will participate voluntarily and of my own free will. I will not hold team leaders, Michael E. Winslett, Baseball 4 Christ USA, Inc. or sponsoring mission/missionaries responsible for any accident, injury, illness or other personal loss that might result from this trip. I authorize team leaders, as my agents, to consent to any emergency treatment that is necessary in the case of accident or illness, which is deemed advisable. I will submit to team leadership and maintain a cooperative spirit in all activities. To the best of my ability, I will participate in the trip and preparation and evaluation sessions. The team will be participating in outside activities, which include but not limited to, the following: baseball, walking, swimming, snorkeling, zip lining, and light construction work. H there are restrictions to any of these activities, please make team leaders aware before travel date. Baseball 4 Christ USA, Inc. team members are expected to have appropriate behavior and social media that remains consistent with team values and beliefs. I give Baseball 4 Christ USA, Inc. permission to view all my social media.

• Follow leadership Instructions at all times. • No inappropriate social media that may be disrespectful to Baseball 4 Christ USA, Inc. • After curfew/lights out -no one outside their assigned rooms. • No student may go anywhere without adult supervision, adult permission or without an interpreter. • No alcohol, foul language or behavior that reflects negatively against Baseball 4 Christ USA, Inc. • No tobacco of any type. • Maintain a cooperative spirit In all mission activities. • To the best of your ability, participate In all mission activities. • Swim Suit -No underwear exposed.
Failure to obey these guidelines may result in the student being removed from the team. Items noted In Red will result in immediate removal from Baseball 4 Christ USA, Inc. and returned back to the U.S. at student's expense.
